Health Declaration

You have applied for a course which requires the satisfactory completion of a medical fitness assessment. This is carried out by our NHS Occupational Health partners. To ensure that you meet the health requirements of your course it is essential that you complete the online health assessment before you enrol and comply with any recommendations, including attending appointments where these are required.

Please Note: The questionnaire will contain a section on immunisations.  Please enter ‘no’ for each of the questions within this section as it does not relate to the course you are undertaking.

We are aware that you will all receive an automated message once you have completed this questionnaire. The following message says;

Thank you for completing the online health assessment. You will be contacted shortly by the occupational health department for your Higher Education Institute (HEI). IMPORTANT: Please refer to the letter from your University regarding the vaccination documentation required. It is imperative you attend all Occupational Health appointments when invited. Failure to do so may result in you being unable to progress your studies.

Please be aware that this is a standard message given across all questionnaires and across all courses. As part of your teacher training course you do not need to have any vaccinations and will not receive a letter from Sheffield Hallam University based on any vaccinations. We understand that this could cause confusion and wanted to let you know that this section about vaccinations doesn’t apply.

Please follow the instructions below to access the online questionnaire. Should your declaration have any gaps or information which the Occupational Health Service needs to investigate further, they will either;

    • phone you to seek further information, or
    • send you a text message with an appointment with Occupational Health to discuss any issues. It is important that you attend your appointment as your enrolment may be delayed if the clearance hasn’t been completed. If you are unable to attend at the given time, you should contact Occupational Health on 0114 271 4737 to arrange an alternative time. Where your declaration doesn’t throw up any issues, you will not hear anything from them.

Instructions for completing your online Occupational Health questionnaire

To complete your Occupational Health questionnaire go to www.healthclear.co.uk. You will be required to enter your username and password.

Username

Your username is the following format – your ‘FIRSTNAMESURNAME‘ – ie JOSEPHBLOGGS  (with no space between the firstname/surname – the information entered is case sensitive, and capital letters are normally required throughout).

Password 

Your password is your date of birth in the following format ‘ddmmyyyy’ ie if your birthday is 1st May 1992, then enter ’01051992′.

Once you have logged in, you will be required to reset your password. An online help guide is also available – just click on the ‘help’ button at the bottom of the screen when you have logged in:

  • It is extremely important that the details you provide are accurate.
  • If you have navigated away from the questionnaire, click on ‘My account’ and ‘Begin questionnaire’ to return to the questionnaire.  DO NOT use your browser’s page forward and page back buttons as you may lose some data which you have entered.
  • The health assessment does not need to be completed in one go, as you can login at any time.
  • The website will indicate to you which sections of the required elements are missing by using the following colour coding system, in the top right of the screen: Blue = not started. Red = incomplete section. Green = completed.
  • You will not be able to submit the health assessment until all of the required elements are completed.
  • Once you have submitted your assessment you will not be able to change any of the details, but you will be able to login at any time to access your questionnaire and certificate of completion. Important note: You are required to keep the University informed of any condition affecting your ability to undertake the course and practice in the profession on completion of the award. This information will be treated as confidential. Occupational health screening can be arranged if issues of concern arise during your period of study with us. Non-compliance with occupational health requirements at any point may prevent you from continuing with your studies.
  • If your health changes before you start your course, or you realise you have made any errors when completing the assessment, it is essential you inform Occupational Health on 0114 271 4737.
